All patients are seen for an initial evaluation upon referral from the physician. Treatment and goals are established based upon the outcome of the evaluation. Physicians receive copies of the evaluation and monthly progress reports.
Referrals - Monday through Friday, 8 a.m. to 4:30 p.m., Weekends, 8:30 a.m. to 12:30 p.m.
