Consultant Dietitians

Mercy Home Care

  • A dietitian may provide an in-home visit as part of the interdisciplinary services provided by Mercy Home Care.
  • Visits may be by patient request, physician referral or by request by other services currently in the home (nurses, social worker, physical therapist, occupational therapist)
  • The visit may be to complete an over-all nutritional evaluation, or to provide diet education regarding specific disease states.
  • The dietitian provides monthly tube feeding follow-up calls with home visits as needed.

Case Management Program for the Frail Elderly/Elderly, Ill/Handicap Waiver Program

  • The Elderly Waiver program is designed to help individuals stay in their home as long as possible by providing services as determined necessary by an interdisciplinary team.
  • Dietitian services are provided to the surrounding counties: Hancock, Winnebago, Cerro Gordo, Worth and Floyd.
  • In-home nutrition counseling is provided as an ongoing waiver service. The dietitian visits to provide evaluations and diet education to assist with over-all health maintenance.
  • Individuals may be eligible for other nutritional services such as nutritional supplements, Meals On Wheels and other meal programs through the Elderly Waiver program.
