Health Information Management
The Health Information Management (HIM) department is responsible for maintaining an accurate medical record for each patient of Mercy Medical Center-North Iowa and Mercy Clinics. Medical records are created and stored in both paper and electronic form.
State and federal laws (HIPAA) govern access, use and release of medical information. To better understand how Mercy Medical Center – North Iowa uses your information, please review our Privacy Notice. This notice is available in paper format at all Mercy locations. As a patient, you will be asked to review this document and sign an acknowlgement of receipt upon admission or physician visit at our hospital or clinics (effective April 14, 2003).
As stated in our Privacy Notice, you have the right to request copies of your medical record (some exceptions apply, as governed by State and Federal laws). To obtain personal copies of your medical record, you will be required to fill out a patient release authorization form. You may call or stop at the HIM Department or any Mercy Clinic to obtain this form. Charges for personal copies will apply. Transfer of specific records for treatment purposes will be processed without charge (exceptions may apply).
