Esophageal Manometry

Esophageal manometry (EM) is a diagnostic test that measures the inside strength of the esophagus and the coordination of muscle contractions within the esophagus. This is done by assessing the esophagus while the patient swallows small sips of water. EM is indicated for patients with difficulty passing food through the esophagus in to the stomach. It may be appropriate in the evaluation of unusual chest discomfort. This test also is done to evaluate chronic heartburn and the surgeon may be considering surgery as an alternative to continued medication.

A small flexible catheter is placed through the nose into the stomach. This catheter contains miniature pressure transducers (measuring sensors) that directly record contractions exerted on the esophageal wall or sphincters. Sphincters are ring like bands of muscle fibers that close a passage or opening.

If your test is scheduled for 7 a.m., you must not eat or drink anything after midnight. If your test is scheduled at 1 p.m., you may have a liquid breakfast before 7 a.m. You must not eat or drink anything after that time.

What happens during the procedure?
The procedure is performed while the patient is awake and without sedation. Medication may interfere with swallowing and have a direct effect on motor events. A small catheter will be inserted through the nose while in a sitting position. When the catheter is inserted, you will be asked to lie quietly on your back with your hands at your sides.

Contraction measurements of the esophagus will be taken through the catheter as the tube is moved through the esophagus and esophageal sphincters. You will be asked to take deep breaths (this determines catheter placement) and take small swallows of water. During swallows, the catheter will measure the strength and effectiveness of the esophagus.

The length of time spent in the Endoscopy Department in preparing for the manometry and the actual exam is approximately one hour. Complications are extremely rare.

What happens after the procedure?
No special or limiting restrictions will follow this examination. Resume your regular diet and activities. Resume your regular medication unless otherwise instructed by your doctor.
