Preferred Health Choices (referred to as “Health Choices”) is a third party claims administrator for large employers who self-fund their health insurance.  Administrative resources are provided by Medical Associates Health Plan.

Unless the address on the card also references Midlands Choice.  Then send it to the Midland’s Choice address on the card. 

Claims must be submitted within 180 days from the discharge date/date of service.
