Coventry offers three products with which we participate:
- Coventry HMO
- Coventry PPO
- Coventry Rental Network
First Health is a wholly-owned subsidiary of Coventry Health Care. Coventry acquired First Health on January 28, 2005. First Health has three basic business lines built upon provider networks. These are:
- First Health Direct (pays Coventry PPO rates)
- TPA/Carrier Business (pays rental network rates)
- HealthCare Preferred is TPA/Carrier
- Workers' Compensation (generally pays Coventry PPO rates)

Who do I contact for eligibility, plan benefits, claims payment, and precertification?
Call the telephone number located on the member ID card.
We do not participate with Coventry's Medicare replacement products that are marketed under the name, Advantra Freedom.
Billing Information:

Coventry does not pay claims billed under midlevel providers (ARNP/PA). All claims for services provided by midlevel providers must be submitted under the supervising physician's name.

Resident physicians must also be billed under a supervising physician. Coventry will not recognize Resident Physicians, even when moonlighting as licensed independent practitioners. Providers must have successfully completed a residency program in the specialty in which they will be listed.

Coventry will not pay facility fees billed under 510 revenue code as they consider them a "clinic charge being billed within a hospital setting."
