I can't remember my username/password now that I have created one.
The NextGen portal provides the opportunity for you to reset your own password. First, click on the link where it says, “I forgot my user name and/or password”.

This will bring up a new screen. Enter the email address that was provided to the clinic or username if known and select continue.

If you entered the correct email address/username, an email will be sent with a link to reset your login credentials. Make note of the username exactly as it appears within the email and click on the “Reset Password Now” link as shown below.

Enter the answer to the security question that populates and select “Continue to Reset Password”.

If the answer typed in does not match what is on file a prompt will appear to try again. If the system will not accept the answer or if the answer is unknown, please contact the clinic.

Enter a new password into the “New Password” and “Confirm Password” fields and select “Save and Enter Portal”. Make sure to include all the required password elements.
