All
appointments are recommended to be scheduled in advance. All appointments
for routine care should be made, as far in advance as possible, since
your health care provider allows for open time frames on their schedule
for urgent medical needs.
On occasion, your provider may be delayed caring for patients with urgent or multiple medical needs. You will be informed of these delays and given the option to wait or reschedule your appointment.
If it becomes necessary for you to cancel an appointment, please notify us as soon as possible, so that we can offer your appointment to another patient.
If
an emergent medical problem arises, please call our office first, if
at all possible before coming in, because directing you to the closest
hospital for emergent specialized care based on your symptoms, may be
the best plan of care.
When scheduling an appointment, please provide the following information:
- Full name of patient to be seen
- Address and phone number
- Date of Birth
- Health care concerns you want addressed at this visit
- Type of insurance (primary & secondary)
By providing this information to us, you will help our health care team render safe and explicit high quality patient care.
