For those who need continued intravenous medication therapy after hospital discharge, Mercy Home Care provides service to be rendered in the comfort of the patient's home. Whether therapy is temporary or chronic, the IV pharmacy staff will individualize medication therapy to each patient.
Home Infusion Pharmacy provides intravenous therapy for all types of patients ranging in age from newborns to the elderly. The pharmacy team provides life-long IV therapy for nutritional needs while temporary therapy may consist of antibiotics, fluids for hydration, chemotherapy or pain management.
The pharmacist develops a care plan as each patient begins therapy and continues to monitor each patient throughout the course of treatment. The pharmacist is part of an integrated team along with the registered nurse, social worker and dietitian. Through the communication of the health care team, individualized intravenous home therapy is provided to each patient.
Respiratory therapy can also be provided in the patient's home. Mercy Home Care and Home Infusion Pharmacy offer a program for patients with respiratory illnesses such as chronic obstructive pulmonary disease (COPD), asthma, emphysema and bronchitis. These conditions may be treated with nebulizer therapy. If a physician orders a nebulizer, the Home Infusion Pharmacy can provide the products and medication the patient will need.
