Going Home

Planning Your Discharge

A multidisciplinary team of physicians, nurses, social workers, chaplains, dietitians, and many others are available to assist you in planning follow-up care at home or at other health care facilities. Your discharge plans and instructions for continued recovery will begin the day you are admitted.

Home Care

Going home from the medical center may require extra planning, especially if you need additional care at home to aid in your recovery. Mercy Home Care, a Medicare certified home health agency, is a department of Mercy Medical Center – North Iowa, and is a full service agency. Professional nurses, licensed therapists, personal aides, homemakers, and companions can support your care at home up to 24 hours a day. Mercy Home Care offers home infusion therapy, enterostomal therapy, and nutrition therapy. Mercy Home Care can provide you with a hospital bed, a wheelchair, or other medical equipment or oxygen services at home after your discharge. Pharmaceutical Care

During your hospitalization, all your medical needs are filled by the inpatient pharmacy. When you are ready to go home, your physician may prescribe new medications to help in your recovery. For your convenience, a Mercy Family Pharmacy is located on both the East Campus (641-428-6100) and West Campus (641-428-5630). Your physician or nurse can call or FAX these prescriptions to the pharmacy in advance and ask that these be ready for pick-up upon discharge or you may stop and fill your prescriptions before going home. If you live in Mason City, there is no need to wait. If you choose, prescriptions may be delivered to you at home at no extra cost. Refills also may be obtained through our pharmacies with easy pick-up, delivery, or mailed free to your home. If you live on the east side of Mason City, you may choose to fill your prescriptions at the Mercy Family Pharmacy-Regency (641-428-6940), located in the Mercy Building in Regency, 621 South Illinois, Mason City, IA. Or if you live south of Mason City, you may utilize Mercy Family Pharmacy-Sheffield (641-892-4640).

Upon Leaving

Before leaving the medical center, please check all drawers, the closet and the bathroom for personal belongings. Any medications being stored for your will be returned. You also need to retrieve any valuables, which were locked in the safe. A member of the nursing staff will accompany you from the room to your car.
