FLEXIBLE BENEFITS PROGRAM


The Mercy Medical Center - North Iowa (MMCNI) basic benefits package is provided as a flexible benefits program to full-time and part-time colleagues with a budgeted work schedule of 32 hours per pay period or more. With the flexible benefits design, colleagues are provided the opportunity to select benefits and coverage’s (medical, dental, life, etc.) that meet their individual and family needs.


Medical/Pharmacy 

The cost is shared by MMCNI and  colleague  

First of the month following 30 days of employment  for regular full-time and part-time benefit eligible colleagues

Participation in one of the three plans is offered:  
• Traditional PPO
• Health Savings PPO
• Essential PPO

Discount offered when you use MMCNI pharmacies and Mercy Home Therapy Shoppe.


Dental 

The cost is shared by MMCNI and  colleague 

First of the month following 30 days of employment  for regular full-time and part-time benefit eligible colleagues

MMCNI provides coverage under two different dental plans. You may elect to participate in one of the following plans:
• Delta Dental Premier High Plan
• Delta Dental Premier Standard Plan


Vision

Colleague pre-tax salary reduction

First of the month following 30 days of employment  for regular full-time and part-time benefit eligible colleagues

MMCNI provides coverage under two different vision care plans. You may elect to participate in one of the following plans:
• UnitedHealthcare High Plan
• UnitedHealthcare Standard Plan


Health Care Reimbursement Account

Colleague pre-tax salary reduction

First of the month following 30 days of employment  for regular full-time and part-time benefit eligible colleagues

Pre-tax salary reductions to reimburse eligible medical, dental and vision expenses. Colleague may elect to contribute pre-tax dollars each pay period up to a maximum of $2,500 annually.


Dependent Care Reimbursement Account

Colleague pre-tax salary reduction

First of the month following 30 days of employment  for regular full-time and part-time benefit eligible colleagues

Pre-tax salary reductions to reimburse colleague for eligible dependent care expenses. Colleague may elect to contribute pre-tax dollars to the account each pay period to a maximum of $5,000 annually.


Basic Life Insurance, Accidental Death/ Dismemberment (AD&D)

MMCNI

First of the month following 30 days of employment  for regular full-time and part-time benefit eligible colleagues

MMCNI provides Basic life coverage equal to 2x the colleagues' base annual salary for both basic and accidental death and dismemberment insurance (AD&D).


Colleague Supplemental Life, Spouse and Dependent Life

Colleague after-tax for supplemental life, after-tax salary reduction for spouse and dependent life

First of the month following 30 days of employment  for regular full-time and part-time benefit eligible colleagues

Colleague may elect to purchase additional term life insurance up to 5x colleagues' base annual salary, subject to plan maximums and medical evidence of insurability requirements. Colleagues may also elect to purchase spouse & dependent life insurance from a number of options.


Group Legal Plan

Colleague after-tax salary reduction

First of the month following 30 days of employment  for regular full-time and part-time benefit eligible colleagues

The Group Legal Plan, sponsored by Hyatt Legal Plans, provides coverage for legal advice and assistance for most personal legal matters (employment and business-related matters are excluded from coverage). The Plan does not impose waiting periods or co-payment requirements.


Short-term Disability (STD)

MMCNI

First of the month following 30 days of employment  for regular full-time colleagues

After a seven day waiting period, payment of 60 percent base weekly wage up to 26 weeks for sickness or accident. PTO hours must be used to satisfy a waiting period and may be used to supplement STD benefits up to 100 percent base weekly wage.


Long-term Disability (LTD)

MMCNI

First of the month following 30 days of employment  for regular full-time and part-time benefit eligible colleagues

After 26 weeks or exhaustion of short-term disability benefits, payment of 60 percent of base monthly wage to a maximum of $10,000 per month. Employee option to buy up to 66 2/3%. Benefits are payable to age 65 or longer, depending upon your age at time of disability.


Paid Time Off (PTO)

MMCNI

Date of hire regular full-time and part-time colleagues

Paid Time Off (PTO) combines vacation, personal and sick days into a single time off benefit bank for use as PTO. You earn PTO on the basis of length of active service and hours worked.

Holidays

MMCNI

Date of hire regular full-time and part-time colleagues

Six (6) paid holidays per year: New Years Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving, and Christmas. Holidays for part-time colleagues are prorated.


Float Day

MMCNI

Date of hire regular full-time and part-time colleagues

One Float day per year may be used to take time away from work with your manager’s prior approval.  This is a “use or lose” benefit.  Prorated for part-time & modified full-time colleagues based on colleagues budgeted hours.


Tuition Reimbursement

MMCNI

After six (6) months employment for full-time and part-time colleagues

Educational assistance is available to any colleague working a minimum of thirty-two (32) hours per pay period and employed at least six months. Budgeted dollars available for reimbursement may vary from year-to-year. 

Educational Loan – Educational assistance is available to any colleague meeting the requirements for an educational loan. To be eligible, the colleague’s anticipated degree must be in a program specifically identified through the annual human resources planning process.


Bereavement

Receive up to 3 days of paid compassionate leave for an immediate family member.


Jury Duty

Date of hire

Jury duty pay will be calculated at your current rate of pay, excluding shift and weekend differentials.


Adoption Assistance

MMCNI

First of the month following 30 days of hire for regular full-time and part-time colleagues

The plan will reimburse the colleagues up to $3,000 per child for eligible expenses that are incurred during the adoption process. Payment of the adoption claim(s) is made after the child has been lawfully placed in the colleagues' home and documentation has been submitted to Human Resources.

Trinity Health Pension Plan

Defined Benefit Cash Balance - All colleagues, regardless of employment status (FT, PT, and PRN) are included in the Trinity Health Retirement Program. You must work 1,000 hours each calendar year to qualify for vesting service credit for that calendar year. Once you have five calendar years of service in which you work 1,000 hours or more under the retirement plan, you are vested with the retirement program.


403(b) Retirement Savings Plan

Colleague may participate in a voluntary tax deferred payroll deduction to the Retirement Savings Plan. The service provider utilized is Transamerica Retirement Solutions.
