Clinical Provider and Diagnositic information:
Family Support - Experienced family navigators are available to help family members with problem-solving, navigating the complex service system, and providing emotional support.
Care Coordination - Our staff help families arrange for the services they need as conveniently and effectively as possible. Many of these services are located in local communities.
Clinic Services - We hold clinics for children and youth who have chronic physical, developmental, emotional, or behavioral problems. A nurse practitioner and staff nurse are the on-site providers. If needed, consultations can be arranged with a nutritionist, and other appropriate care providers via telehealth. Recommendations, including referral to community resources, are always planned with the child and youth’s family.
Systems Building (Infrastructure) - We also work to improve the health care system. For example, we work to assure that all children and youth with special health care needs have a medical home. We also advocate for family-friendly policies, monitor the quality of statewide services, and survey families about their needs.
